Caledonian Practice Development Model

Our practice development model seeks to create a driving force moving current practice towards evidence based practice described within the Best Practice Statements. Through a range of integrated and systematic facilitation and knowledge sharing activities, we aim to fuse the scholarship of practice with the scholarship of inquiry. Intertwining scholarship in this way drives practice forward, enabling integration of the diverse evidence base, related learning and application to practice. The process is developmental and perpetuated through the momentum generated by each Community of Practice.

The knowledge synthesis undertaken in preparing best practice statements is an illustration of the unique approach to integration within this project. Learning is continual and critical to our pragmatic approach to application and testing within the Demonstration Sites. As the spiral matures, application moves beyond the demonstration site to the practice of the link nurses and through dissemination and publication of the Best Practice Statement to the wider practice community. Implementation support continues through the Better Care Project and flexible CPD which establish new ‘implementation communities of practice’ and so it is hoped momentum continues.
